Fees & Insurance

I currently am in network with the following plans:

• Optum (United Healthcare)

• Aetna

• OhioHealthy

• Medical Mutual

• Anthem Blue Cross and Blue Shield

• Cigna

My out of pocket cost is $120 per 50 minute individual session.

It is your responsibility to verify your benefits prior to our time together. You can do this by calling your insurance provider and ask about your outpatient mental heath/behavioral health benefits. For their reference, my NPI number is 1104258649. Any co-pays or services not covered by your insurance will need to be paid in full at the time of service and billed to your card on file at the end of session.

Cancellation

Cancellations must be made 24 hours in advance of our scheduled session time. Any session not cancelled 24 hours in advance will be charged at the full hourly rate.

Superbill

If you are interested in utilizing your insurance providers out of network benefits, I can provide you with a Superbill statement after our session. You can then submit that to your network for reimbursement. Any cost associated with care will be do up front, at the time of service.

I currently provide telehealth therapy on Monday, Tuesday, Wednesday, and Thursdays and have immediate openings.
